If you want an easy way to capture deficiency information (ie. description, photos, and status) on your form submissions, you'll need to activate deficiency triggers in the form builder for each field.
Here are the steps to activate deficiencies:
Go to the My Forms page
Click the Customize bottom on the form you want to add deficiencies to
Select the field, then click the gear icon
Check the Capture Deficiencies option in the right panel
Lastly, select the Failing Value.
Once the deficiency triggers are set up, they will immediately be active on your next and future submissions.
